Preparing for Your Appointment
Your Take Charge! binder is a useful tool to collect and organize information between appointments.
Keep a running list of questions to discuss with your provider. Keep the list in your binder and add to it between appointments as questions arise so we can address everything.
This form is in the Medical section of your Take Charge! binder, and should be completed within 10 days of each appointment.
Bring all your prescription bottles to every appointment. It is not necessary to bring supplements and over-the-counter medications, however it is important to keep these updated on the medication list in your Take Charge! binder.
Blood tests, EKGs, brain imaging studies, and office notes from your primary care physician or a specialist can be useful to your MMC provider.
Bring copies of completed or revised Advanced Directives to the appointment so we can enter them into the medical record.
Please bring new insurance information, changes to pharmacy, primary care physician, home address, or contact information.
To add a new Care Team member to the Patient Portal, bring the name, phone number, address and email of the person. They can also be added to the MMC mailing list by clicking here.
Our appointments are longer than traditional medical appointments, to allow time for questions and discussion.
- First appointment: 2 hours
- Follow up appointments: 1 hour
- Injection/Biopsy appointments (Movement patients): 30 minutes
We have a responsibility to protect the personal health information (PHI) of all our patients. Smart phones, tablets, and other video and/or audio recording devices have the potential to put this information at risk. Therefore:
- Audio and/or video recording in any common area of the clinic is not permitted unless it is required for communication purposes in accordance with the Americans with Disabilities Act (ADA) or other state/federal access to healthcare laws.
- While in a patient room with a provider, the use of any audio and/or video recorder is not permitted unless it is required for communication purposes in accordance with the Americans with Disabilities Act (ADA) or other state/federal access to healthcare laws.
- Recording of a telehealth visit is not permitted.
- No recordings of a patient visit in any format may be shared at any time on any social media platform.
