An Annual Check-In
A Care Plan visit is an annual one-hour appointment for patients with cognitive impairment or dementia. This visit includes the primary caregiver, a Memory & Movement nurse practitioner or physician assistant, and a social worker. The patient does not typically attend this appointment. (automated appointment reminders may not make this clear)
Following this visit, a written Care Plan is provided to add to the Take Charge! binder for easy reference.
