A Care Plan appointment is an opportunity for candid conversation and questions in a safe space.

This annual check-in is for caregivers of patients with cognitive impairment or dementia. This visit includes the primary caregiver, a Memory & Movement nurse practitioner or physician assistant, and a family care navigator. The patient does not typically attend this appointment. (automated appointment reminders may not make this clear)

Following this visit, a written Care Plan is provided as a reference tool and record of the conversation.

Care Plan appointments are led by a Family Care Navigator

In addition to facilitating Care Plans appointments, Family Care Navigators work with all MMC clinical teams to connect caregivers and families with the support and resources they need throughout the year.

Bianca is originally from Charlotte, but spent 2.5 years in McLean, VA serving as a Geriatric Care Manager and Unit Leader for the Northern VA unit of Aging Life Care Association. She's held multiple roles in aging services, health and social care settings.

Bianca has a Master of Science in Health specializing in Geriatric Management and a Bachelor of Science in Public Health. She is a Certified Dementia Care Partner, and plans to sit for the Certified Care Manager exam this year.

Courtney earned a Bachelor of Arts degree in psychology from Meredith College in 2006 and a Master of Social Work (MSW) from The University of North Carolina at Charlotte in 2010. A natural people person, Courtney was drawn to the time and attention model of Memory & Movement Charlotte and is passionate about helping patients and their caregivers.
