Care Plans

An Annual Check-In

A Care Plan visit is an annual one-hour appointment for caregivers of patients with cognitive impairment or dementia. This visit includes the primary caregiver, a Memory & Movement nurse practitioner or physician assistant, and a family care navigator. The patient does not typically attend this appointment. (automated appointment reminders may not make this clear)

Following this visit, a written Care Plan is provided as a reference tool and record of the conversation.

What's Discussed?

  • Diagnosis and treatment
  • Main concerns and goals
  • Advance Directives
  • Home environment
  • Safety assessment with recommendations
  • Strategies for behavior, personal care and communication
  • Caregiver stress
  • Community resources
  • Alternative living options
