A Care Plan appointment is an opportunity for candid conversation and questions in a safe space.

This annual check-in is for caregivers of patients with cognitive impairment or dementia. This visit includes the primary caregiver, a Memory & Movement nurse practitioner or physician assistant, and a family care navigator. The patient does not typically attend this appointment. (automated appointment reminders may not make this clear)

Following this visit, a written Care Plan is provided as a reference tool and record of the conversation.
